The effect is to indirectly increase intracellular Ca2+ concentrations in certain cells, particularly myocardial cells. Therapeutically, this both enhances cardiac inotropy (contractility) and slows the heart rate. However, excessive elevation of the intracellular Ca2+ also increases myocardial excitability, predisposing to the development of ventricular dysrhythmias. Enhanced vagal tone, mediated by the neurotransmitter acetylcholine, is common with poisoning by these agents, and produces bradycardia and heart block.
